We are not a medical provider and do not diagnose, treat, or prescribe.
All laboratory tests are performed by certified partner laboratories. All medical decisions and follow-up actions should be discussed with a licensed healthcare provider.
3. Eligibility for Services
To use our services, you confirm that:
You are at least 18 years old or have parental/guardian consent
You provide accurate personal and medical information
You understand that certain tests may require a physician’s order
4. Scheduling & Appointment Policies
4.1 Appointment Requirements
Appointments must be scheduled in advance. Precision Lab Solutions may request additional information to confirm your appointment.
4.2 Missed or Late Appointments
If a client is more than 15 minutes late, the appointment may be:
Rescheduled
Subject to a cancellation or no-show fee
4.3 Cancellations
Cancellations must be made at least 24 hours in advance.
Late cancellations may incur a fee to compensate for the phlebotomist’s reserved time.
5. Payment & Billing
Payment is due at the time of service unless otherwise stated.
We accept:
Major credit/debit cards
Approved insurance (if applicable)
HSA/FSA cards
Online invoice payment
5.1 Refunds
Refunds are not issued once a specimen has been collected and delivered to a lab.
If service could not be completed for reasons beyond your control, refund requests may be reviewed on a case-by-case basis.
6. Laboratory Processing & Results
Precision Lab Solutions:
Collects and transports specimens
Coordinates with certified labs
Provides result delivery through secure portals or email (as allowed)
We do not control:
Lab processing times
Lab result delays
Lab test interpretations
Any concerns regarding medical interpretation should be discussed with your healthcare provider.
7. Accuracy of Information
You agree to provide accurate, current, and complete information when scheduling or using our services.
Precision Lab Solutions is not responsible for delays or errors caused by incorrect information.
